Dental Network Highlights Enrollment/Eligibility Member Services Quality Assurance Renewal Policy Commonly Asked Questions Q: How many times a year can I have my teeth cleaned?A: Teeth cleanings are covered at the price listed in the Schedule of Benefits twice per year. Q: What qualifications determine if a dentist is eligabel for our Provider Panel? A: Dentist must meet our highly selective credentialing process which is based on education, backgroud, location, office facilities, and other requirements. Q: My dentist isn't currently on you Provider Panel. What can I do? A: Just tell us! When you let our Member Services Department know who your dentist is, we will contact him/her about becoming a Provider. Q: What type of emergency coverage do I have? A: In case of an emergency, a Member should contact their usual network Provider. Q: Can this Plan coordinate benefits with other insurance Companies? If so, How? A: Yes, this Plan can coordinate benefits with other insurance companies. In order to receive benefits from this Plan, you must see one of our Providers. After paying the reduced fee, you will need to file a claim form with your other insurance for reimbursement. Exclusions: Services for injuries or conditions that are covered under Worker's Compensation or Employer's Liability laws. Services which are provided with cost to any member by any municipality, county, or political subdivision. Services which, in the opinion of the attending dentist are not necessary for the patient's health or that cannot be performed because of the general heath condition of the patient. Hospitalization, general anesthesia, an I.V. sedation. Cost of dental care which is covered under automobile, medical, no fault, or similar type insurance. Limitations: Work in progress. Work in progress after enrollment on the dental plan bust be completed by dentist who started work. Any dental procedures performed be a nonparticipating dentist are not covered. Careington cannot guarantee the continued participation of any dentist. If he/she leaves the plan, you will need to select another dentist. Not all types of dentist may be available in you area. Some Providers may charge for missed or broken appointments if no prior notice is given. It is the Member's responsibility to verify that the dentist is a participating Provider. Disclaimer: This plan does not cover all procedures which might be provided by a general dentist. Any procedure delivered which is not listed on the Schedule of Benefits may cause additional cost to be incurred which is the responsibility of the Member. The dollar amount specified adjacent to each procedure may not be the only cost incurred for a given treatment because the may require more than one dental procedure.
